State of Georgia,
Banks County.
Personally appears T. A. Carlan of said County, State
of Georgia, who being sworn, says on oath that he was born on the 27th
day of March 1839, that he is a bona fide citizen and resident of Georgia, and
has been continuously since birth 27th day of March 1839, that he
enlisted in the military service of the Confederate States (or of the State of
[blank]) on the 1st day of Oct. 1861, during the war between the
States, and served in Company B of 29th Regiment of Ga. Volunteers
Wilson’s Brigade, and was honorably discharged on the 17 day of Jan. 1865; that
whilst engaged in such military service, and in line of duty in the State of
Tennessee, on the 7th day of Dec. 1864 he was disabled or wounded as
follows:
“At Murfreesboro in Tenn. I was wounded in the right
thigh between the knee and thigh joint which caused the thigh to perish
away. The Doctors in attendance said
the ball that inflicted wound was a poisoned one. Since the 1st of April last I have had to resort to
Artificial means to walk with the aid of crutches to walk at all, the above
wound the cause of my total disability, the thigh above mentioned becoming
weaker each year.”

At Greensboro, N. C., April 26th 1865.
Was applicant present?
Was not.
If not, where was he?
At home on furlough.
How come there?
Was wounded and sent home.
And by whose authority? State fully.
Was sent home on 60 days furlough by officers in
charge.
Deponent desires to participate in the benefits of
Section 1250 of the Code, and the Acts amendatory thereof, and makes
application for the pension to which he entitled for the year thereunder,
ending October 26th, 1902. [This is a form from 1902 used over the
years.]
