Pension Office 4/25/1898
The witnesses will separately state when and where
they first noticed the disease mentioned.
Was the deceased sick at any hospital, if so, when &
how long did he remain in such hospital?
Was he discharged from service-if so, where and when? Do you speak entirely from personal
knowledge? Have you testified in this
case before now?
Richard Johnson
Commissioner of Pensions.
Whatever the problems were they were apparently
resolved as a Warrant for pension was issued 8/15/1898.
