For Use of Applicants Who Have Not Heretofore Drawn.

 

State of Georgia,

Banks County.

Personally appears John W. Parker of said Banks County, State of Georgia, who being duly sworn, says on oath that he was born on the 4 day of February 1837, that he is a bona fide citizen and resident of Georgia, and has been continuously since 4 day of February 1837, that he enlisted in the military service of the Confederate States (or the State of ......) on the 12 day of April 1861, during the war between the States, and served in Company A of 2nd Regiment of Georgia Volunteers Benning’s Brigade, and was honorably discharged on the 9 day of April 1865; that whilst engaged in such military service, and in line of duty in the State of Virginia, on the 4 day of Oct. 1864 he was disabled or wounded as follows:

I was taken sick in the ditches at Yorktown, Va., which resulted in rheumatism in right leg, hip and shoulder, which I contracted at Yorktown, Va. My limbs swells at times, I cannot feed myself, I cannot walk at times.  I also contracted chronic lung trouble.  I have at times difficulty in breathing, my lung troubles resulting from measles and relapse of same.  These diseases contracted while I was in service which has increased ever since until such disability disabled me from performing any kind of labor.  I have become and invalid on account of these diseases which I contracted in the service.  Theses diseases were caused by extreme fatigue and exposures while in service.

Where was command surrendered?  Appomatox, Va.

Was applicant present?  He was.

Deponent desires to participate in the benefits of Section 1250 of the Code, and the Acts amendatory thereof, and makes application for pension to which he is entitled for the year thereunder, ending October 26th, 1902.
