Personally appears Joseph Parsons
of Banks county, State of Georgia, who, being duly sworn, says on oath that he
is a bona fide citizen and resident of said State, and has been such since the
3rd day of July 1829; that he enlisted in the military service of
the Confederate States (or of the State of Georgia) during the war between the
States, and served as a Private in Company G of 52nd Regiment of
Georgia Volunteers, Stovall’s Brigade [in parenthesis written in right margin “
Company H 34th Ga. Infantry Regiment and Company G 1st
Regiment Georgia State Troops”]; that whilst engaged in such military service
at the battle of ........ in the State of ....., on the .....day of
December 1863, he was wounded as follows:
Was vaccinated by order of
Confederate Service near Athens, Ga.
Matter was poisonous. Kept
getting worse from that time for near seven years, when it healed leaving
shoulder permanently stiff and perished away-Leaving the shoulder joint
permanently useless, and left arm disabled so as not to be able to perform
ordinary manual labor.
Deponent desires to participate in
the benefits of the Act, approved October 24, 1887, and makes application for
the allowance to which he is entitled thereunder.

Personally came J. K. Thompson, A.
Massey & [Powell A. Waters] citizens of Banks county, in said State, who,
being duly sworn, say that they are acquainted with Joseph Parsons and know
that he received the wounds (or contracted disease) in the military service, as
stated by him in the foregoing affidavit; that said wound (or disease)
permanently disables applicant, as stated by him; that said applicant is a bona
fide citizen of this State, and resides in Banks County, and we are well
satisfied that all the statements in his affidavit are true.

me..........Ordinary of said county, Vincent D. Lockhart and A. H. Stapler,
both know to me as reputable physicians of said County, who, being severally sworn,
say on oath that they have carefully examined Joseph Parsons and after such
examination say that the applicant has been injured as follows:
Permanent stiffness of left
shoulder joint. Joint is perfectly
stiff. Muscles and tendons of upper arm
and ligaments of shoulder atrophied perfectly rigid. As regards disability, will say that left arm is totally useless.
