Affidavit of Mrs. Mary J. Sisk of the death of William S. Sisk

Banks County

            Personally appeared before me ordinary of said County Mrs. M. J. Sisk who duly sworn says on oath that her husband, Capt. W. S. Sisk, departed this life the 22^nd day of February, 1908, and she is his lawful widow.

            That he was drawing a state pension of $60.00 per annum.
