Personally appeared J A Lewallen Jr who on oath says that J A Lewallen was his father, that his said father was on the disabled pension roll of GA and drew a pension of $100 on account of loss of an arm in the service of the COnfederate States, that the said J A Lewallen had lived with him ( J A Lewallen Jr) for the last 6 years and had no wife nor minor children and that he died at his home on Jany 2nd 1908 and that the following is a true and correct account of expenses incurred in his last sickness and for burial expenses to wit:
Coffin and clothes for burial $32.00
Help hired in last sickness $25.00
Medicine and Doctors bill in last sickness $25.00
Total $82.00
